Payment of services not covered by insurance plan
All payments required by the insurance plan will be required at the time of appointment.
Previous Patients
If you were a previous patient dismissed due to owed balances, failure to comply with controlled substance policies, or failure to comply with appointment no-show/cancelation policies, you may not be accepted as a new or returning patient.
